PACE of the Triad

PACE is called the Gold Standard in Senior Healthcare by the American Society of Aging. PACE provides for all the medical needs an older adult could have. Instead of piecing together services offered by different providers — for example, senior centers, healthcare plans, home care and transportation companies. PACE provides it all.

Our team is so coordinated, they meet several times a week to review our participant’s care plans, and make any needed adjustments and changes. It is this coordination of many different professionals — called the Inter-Disciplinary Team — that is the key to PACE’s success.
