Mission: To improve the health and social well-being of the communities we serve through equitable, high quality, patient-centered care that is culturally responsive.
We believe that everyone should have access to high-quality medical and dental care, regardless of ability to pay.
Since 1971 when we saw our first patients in a local elementary school, Fair Haven Community Health Care has been making health care accessible to the communities we serve.
We began our work under the leadership of the Alliance for Latin American Progress, a community advocacy agency, and began seeing patients two evenings a week. With a small group of dedicated nurses, doctors, students, and neighborhood volunteers, the “Fair Haven Clinic” was able to serve adults and children on a walk-in basis for minor ailments, immunizations, and family planning services. Gradually, patients began to consider Fair Haven their medical home and not a place for episodic care.
Today, we provide care for over 32,000 patients at over 130,000 office visits in 18 locations. This includes families who have been part of the Fair Haven Family for multiple generations . We are proud to offer a wide range of primary and specialty care services, as well as evidence-based patient programs to educate patients in healthy lifestyle choices, pre-natal care and parenting workshops. Even as we grow and are able to bring high quality health care to more areas that need access, we continue to put our patients first in everything we do.
